Returning Students

This section is for students who have already enrolled through our office (Gundersen 108). You will still be required to fill out the following forms before being enrolled.

2.  Enrollment Form

3. Student Information Form

4. Testing Center Proctor Agreement Form (if course requires)
     Individual Proctor Agreement Form-must be pre approved by Distance Learning Office
