• Student/Proctor Information Form
• Off Campus Testing Center Proctor Agreement Form if required (see instructor syllabi)
• Off Campus Individual Proctor Agreement Form - must be pre-approved by Distance Learning Office
• Payment Agreement Form (NEW Submittable form) - this MUST accompany the enrollment form
• Review Proctor Policies

Step 4: Enrollment

• New Students – Send in enrollment form (MUST be accompanied by Payment Agreement Form)
  (to fax 405-744-1891 or mail to Distance Learning, 215 School of Business, Stillwater OK 74078)
• Returning Students – Online Enrollment
• More detailed information regarding enrollment

Step 5: Purchase Course Texts

Text Book inofrmation can be obtained by choosing the course(s) that you are interested in.

Step 6: Confirmation of Enrollment

Check SIS to verify enrollment in your class(es).

Step 7: Course Materials, Syllabus, etc.

One week prior to the start of classes:

• Log onto Online Classroom with your okey username and password
• Check your enrollment
• If classes are not listed, call 405-744-4048 or email cepd-dl@okstate.edu immediately.
